Kenpachi has always been a bit different, and it's in the last few chapters of the story that fans get to see his Bankai, which is actually amazing to look at, but also quite unsettling.

Before Barrag joined Aizen's Espada army, he was once a high and powerful king of Hueco Mundo. So while he already looks quite authoritative in his regular appearances, no one expected him to turn into a monster when he awakens to resurrection.
During his fight with Soi Fon, Barragan sheds all his skin and becomes a skeleton with a crown on top of his head and a long flowing cloak that follows him from behind, reminding onlookers of his royal status. Considering that this form literally pops out of nowhere and covers two entire pages of the comic, it's a visual so shocking it's the stuff of nightmares.
4 Yhwach's Soul King Form
Numerous eyes appear on the surface and Yhwach's face is absorbed into darkness.
- First appearance: TYBW episode 27
Yhwach, the self-proclaimed Quincy King who serves as the main antagonist in the final arc, receives this form after successfully absorbing the Soul King's power. Once he does so, Yhwach acquires a black mask that covers the upper half of his face, covered in creepy glowing eyes, gifting him an incredible amount of power, especially when paired with the Almighty.

On the one hand, it appears that Yhwach has been completely corrupted by this black ooze, but in reality, it has always been expected of villains seeking to seize such power, making him virtually unstoppable. Still, it's a decidedly scary sight to behold, and it's enough to send chills down the spine of even the most seasoned horror fan.
3 Fusion of Ayon and Quilge
Quilge absorbs Ayon, fusing it with the terrifying Void.

This was a somewhat baffling form that had many fans scratching their heads when it first appeared, but it was eventually explained that Quilge used Ayon to absorb the Hollow's energy using a technique called Sklaverei. This resulted in a hideous form that merged the features of Quilge and Ayon, with Quilge's wings now fitted with googly eyes and his face appearing more hollow than human.
As a way to introduce the Quincies, Quilge does a very good job of showing just how twisted and unsettling they are, considering they have numerous similar abilities. Ichigo's Hollow form may look a little unsettling, but it's nothing compared to this appearance of Quilge.
2 Ichigo's Vasto Road
Ichigo's Hollow takes over his body and turns him into a sadistic shell of his former self.
- First appearance: Episode 271
The first and only time the Vasto Lorde appears in the series is during Ichigo's fierce battle with Ulquiorra. There he meets his end at the hands of the Espada, who spend much of their time tossing Ichigo around like a rag doll. After punching a hole in his chest, seemingly killing him, Ichigo's Hollow begins to take over him. Just like White Ichigo had predicted many episodes before when he said the “horse” would take over the “king.”
The result is a frankly demonic-looking hollow version of Ichigo, designed with a pair of tattered horns and a face so sadistic that it would keep anyone up at night. The fact that Ichigo's Vasto Lorde form was able to easily overpower Ulquiorra and viciously attack his friend Uryu also goes a long way in making this Ichigo's scariest form by a country mile.
1 Like Nodt's Vollstandig
Despite his fear of Hell, this form of Nodt looks like it crawled straight out of Hell.
- First appearance: Episode 19
In his base form, Nodt is already the creepiest of the Sternritter, but his Vollstandig takes it to a whole other level. When this technique is used on Rukia, As grows into what can only be described as an enlarged representation of herself, complete with bleeding eyes and a mouth full of fangs.
Considering how callous this form of As looks, it almost resembles a creature straight out of a horror comic, and it only gets worse the longer its abilities last. Nodt was known as a character who was always afraid of being sent to Hell, and this design makes him look like he's a creature that crawled out of the pits of Hell. Luckily, Rukia was able to remain calm and composed when facing this creepy enemy, and was eventually able to defeat him with her Bankai.
